# Employee Review Prep Worksheet

Use with [Prepping Your Own Performance Review With AI, Without Inventing Evidence](/articles/employee-performance-review-prep-with-ai).

## Step 1: Raw evidence (fill this in yourself, before using AI)

| Project / task | What happened | Metric or outcome (only if you can verify it) | Source (email, dashboard, ticket, message) |
| --- | --- | --- | --- |
| | | | |
| | | | |
| | | | |

Feedback received this period (quote or paraphrase, with date and source):

- ___________________________________________________________________
- ___________________________________________________________________

## Step 2: Organize by review category

For each of your company's actual review categories, list only evidence from the table above. Leave empty if nothing fits — do not fill with invented material.

| Review category | Evidence that fits (from above) |
| --- | --- |
| | |
| | |

## Step 3: Draft language check

For each paragraph AI helps you draft, confirm:

- [ ] Every number matches what I listed in Step 1 exactly.
- [ ] Every project name and outcome is real and traceable to a source.
- [ ] No adjective ("significant," "major," "substantial") was added unless I used it myself.
- [ ] Nothing was added to fill an otherwise-empty category.

## Step 4: Final verification pass

For each claim in the finished draft, answer: can I point to the source right now?

- If yes: keep it.
- If no: soften it to what you know for certain, or remove it.

## Disclosure

- [ ] Checked whether my company's review form asks about AI assistance.
- [ ] If yes, disclosed honestly — see [keeping your name on AI-assisted work](/articles/keep-your-name-on-ai-assisted-work).
